Chronic Disease Self-Management Program Lay Leader Training

The peer-led Chronic Disease Self-Management Program (CDSMP) is an evidence-based,
6-week workshop for adults who either have one or more chronic health conditions or are caregivers for those who do.
Program content focuses on a variety of skills/tools such as:
- Stress Management
- Medication Usage
- Better Breathing
- Healthy Eating
- Informed Treatment Decisions
- Communication Skills
- Physical Activity Dealing with Depression
- Goal Setting
- Problem-Solving
- Working with your Healthcare Team
This interactive Lay Leader Training is free and includes the CDSMP Leader Manual,
resource book, “Living a Healthy Life with Chronic Conditions”, and relaxation CD
Location: Southwest Sub-courthouse Voting Center

Fort Worth, TX 76133
Time: 9:00am – 4:00pm
Dates: July 18, 19, 25 & 26, 2022 (must attend all four dates)
